I havent read the actual report. But my view is blaming preventer failure for anything is always a bit tenuous. Sure a preventer may help prevent an uncontrolled gybe in big breeze, but once that uncontrolled gybe has actually happened and the breeze has fully backed the main then something's bound to let go sooner or later, whether it's the preventer or something else.
